Having trouble finding a studio to perform your custom video? Fill out the form below and you will be notified when a studio is willing and ready to fulfill your request.

OZMO we need a form with a small entry box for minimum length and a larger text entry box for the video request description. Immediately underneath the form will be a list of requests that have been made, along with a 'delete' button the member can use to delete or modify his request, an 'accept' button for a studio to click indicating they will make the video, and a 'contact member' button that will allow a studio to contact a member if they have questions about the request.
